Order - Sort order for the disclosures to appear on the application.  

Prompt Text  - Text value that will be displayed. 

Sub Type - Sub type the field will display on. 

Result Dynamic Field - What dynamic field will be shown.  

Is Y N Result - If the disclosure is a Y or N result. 

Is Comment Field Shown - If the comment field is shown. 

Mandatory - Whether the Disclosure is mandatory. 

Actions - Delete action and Results 

Add Disclosure - Adds another tab row.   

Licence Type Disclosure Results 

Comment Field Prompt - Sort order for the disclosures to appear on the application.

Document Category - Document Category the result will display on.  

Document category prompt - Prompt for the document category.
