Order - Sort order for the declarations to appear on the application.  

Prompt Text - Text value that will be displayed. 

Sub Type - Sub type the field will display on. 

Is Comment Always Shown - If the comment field is always shown. 

Is Comment On Yes - If the comment field is only shown when Yes is chosen. 

Is Comment On No - If the comment field is only shown when No is chosen. 

Mandatory - Whether the declaration is mandatory. 

Actions - Delete action. 

Add Declaration - Adds another tab row.
